There’s a new program in Grey Bruce to ensure more elderly patients return home rather than end up in long term care facilities after staying in hospitals.
The Community Care Access Centre has partnered with local hospitals to help bridge the service gaps between hospital and home.
Regional Manager Cathy Kelly notes changes in Ontario’s long term care strategy mean that less people qualify for long term care.
The new regulations are aimed at opening up more long term beds for those who truly need it and are currently in hospital awaiting a transfer.
The CCAC has been charged with the task to help make the transition from hospital to home smoother for those elderly patients.
Within 24 hours of being released from hospital, the CCAC will have a nurse practitioner visit the patient at their home to determine what services they need.
This can include everything from daily visits from a nurse or personal support workers to arrange a meals on wheels program.
There also programs in place to support care givers and give them respite relief.
Kelly says these programs are already in place within the community and are receiving positive feedback from clients and their family members.
Kelly supports the changes.
She says the stay at home programs allow seniors to remain independent longer and also save significant money in health care.
